This Payroll Setup Forms Kit is for companies that currently have an existing payroll or for companies that do not have any payroll this calender year.

To setup your company for payroll processing services
we will need:

• Copy of a company check on the account from which
  we will draft payroll. (May be voided)
• New Client Info sheet
• Signed IRS Form 2848 (State & Local POA's if required)
• Copy of Payroll Services Contract, signed
• Employee Information
• Copies of Current Yr. 941’s, Copies of State and local returns (if applicable)
• Copy of IRS document with Company name and Company EIN, for verification
